An arm of Frimley Heath has been shortlisted for an award for a project that monitors people with long term medical conditions remotely.
Connected Care is part of the Frimley Health and Care Integrated Care System (ICS). Frimley runs hospitals, doctor’s surgeries and other health services in Windsor & Maidenhead and Slough, among other regions.
For the project, patients with long term medical conditions were offered remote monitoring to help them stay well and receive care if they became ill.
Designed for those at the highest risk of their illness getting worse, the monitoring service enables patients to get advice and care when their symptoms suggest they need it.
Technology enabled patients to regularly share symptoms and blood pressure with a team of healthcare professionals able to respond quickly.
As a result, their care was 'better planned and offered when needed’, reducing the need for patients to be rushed to hospital. This also 'improves their ability to live well at home.'
Patients are more in control of their wellbeing - and are reassured that someone is keeping an eye on their symptoms meanwhile.
The project has been shortlisted for the Virtual Care Project of the Year category at the HSJ Partnership Awards, which celebrates improvements to healthcare and collaboration with the NHS.
Now in its seventh year, 228 entries have been received for this year’s awards.
